Your Feedback is Important to Us! Complete Our 2025 Shiawassee County Community Health Needs Assessment

Tuesday, April 1, (2025) – Owosso, Michigan – Memorial Healthcare, in collaboration with the Shiawassee County Health Department, is conducting its triannual Community Health Needs Assessment to better understand the most pressing health concerns in our community. By taking this quick and confidential survey you’ll help shape programs and services that address these needs and improve local health outcomes. If you choose, you can also be entered for a chance to win a $400 gift card bundle from local businesses around Shiawassee County! 

The survey may be taken online here. The survey will end on June 15, 2025.

  • Community District Library (Bancroft, Byron, Corunna, Lennon, Morrice, New Lothrop and Perry)
